Combination of above demanded that left pedal input be at or close to maximum possible As well: Varying combinations of gross weights & density altitudes Of most import: a tail or crosswind existed at the crucial moment Flight maneuvers: Involved in these accidents were observation missions (right handed turns) A Max performance take-off Right downwind turn at low airspeed Steep approaches Go-around ‘’O’ or low air speed maneuvering Pre-flight Planning: Plan your flight and fly your plans. Pre-flight planning determinations advised by: Professionalism; operator’s manual; AR 95-1; and ATM’s. Planning Sequence: For an OH-58A  mission given . Details of mission given and then how to prepare: Appropriate charts should be used depending on the aircraft. For mandatory departure plans. Extremely detailed information given as to how to fill out the ‘charts’. In summary: given the example:  maximum winds under optimal conditions exist; The engine is at 103% power RPM .and that any reduction of engine power RPM below 103% will cause a reduction of tail rotor thrust. Always hover into the wind, and if hovering turns are required, they should be made with smooth and deliberate t/r pedal inputs.  Arrival planning charts are used the same way. Actions at the Aircraft:  To verify the planning chart with the aircraft:  In accordance with task #2002: Perform a hover (power) check If hover torque is at or below predicted torque, PPC is valid. If however, torque is more than predicted torque, or if adverse winds exist, the following should be done: Either: reduce the gross weight; revivify PPC entries or perform  an OGE hover check Hover check: Verifies pressure, altitude and temperature figures Other factors involved in tail rotor failure can be: Maintenance Pilot technique, pilot proficiency, ‘control touch’, and professionalism. Its about sticking with the basics. Conditions in the Air to be aware of:  What are the winds? And where are they? Relative to the aircraft. What are my aircraft’s capabilities? And what are my capabilities? How to Recognize signs of tail rotor loss of effectiveness? Check pre-flight conditions and maneuverability Corrective actions to take when this loss of effectiveness occurs: Although there are no pat answers: You can: reduce collective (as altitude permits) to decrease the torque effect of the M/R And simultaneously reduce  your left pedal input. As the aircraft turns into the wind, apply forward cyclic to accelerate to transnational lift Also, increase collective pitch and apply pedal as required for control & trim purposes.
